Context engineering is empirical. When something goes wrong, look at exactly what the model saw — not just what you intended it to see. Did your system prompt actually get included? Did a retrieved document get truncated? Did old, stale context leak in? Most fixes come from observing the real input, spotting what's missing or noisy, and adjusting. Treat each interaction as a small experiment.
